I certify that I am at least 21 years of age and that I will abide by all laws, rules, and regulations pertaining to this event.

I will hold blameless the Oklahoma City Chapter of the National Leather Association, its members, event sponsors, venue operators, officers, agents and, or assigns associated with this event from any and all liability for any damage or injury to my property, to myself or my reputation of any kind as a consequence of my attendance at this event.

I certify that I am not an employee or agent of any local, state, or federal law enforcement, media agency or conservative religious organization, or that if I am, I am attending this event solely for my personal enjoyment and not as part of any investigation.

I certify that I am attending this event for my personal enjoyment and that I understand that in so doing I will be exposed to many adult-related activities that some people might find offensive. I hereby certify that I do not find such activities offensive and that I consent to being exposed to such activities at this event.

I understand that if I do find something to offend me, it is my responsibility to leave the area without disruption or hesitation.

I understand and agree that the some activities I may participate in may involve risk that may result in injury and/or loss. I acknowledge that I am responsible for covering the cost through my own insurance or out of pocket expense of any medical attention I may need as a result of any activities I engage in during any Tribal Fire event. I understand that I will be held accountable for my own actions as an adult.

I understand the limited use of photographic and recording equipment must be cleared through the Tribal Fire organizing committee. New for 2004 - Because of the high number of cell phone with cameras, NO CELL PHONES ALLOWED IN WORKSHOPS OR PLAY SPACE.

I understand and agree to dress appropriately in areas where there may be non-event persons. These areas include but are not limited to the hotel lobby, restaurant, bar, courtyard and hallways. At a minimum, I will keep my genitals covered including a 2-inch strap up the back and (women) have my nipples/aureoles covered.

I understand that NLA-OKC and Tribal Fire do not support the use of illegal substances. I agree not to bring any such substances to any Tribal Fire event. Should I do so, I understand that I will be removed from the event and lose all weekend privileges with absolutely no refund.

The NLA-OKC reserves the right to approve or reject any application. You must present a valid ID at registration check-in.

Your ID must have your name as listed on your application as well as a date of birth certifying you are at least 21 years of age.
